Areas to work towards

The below is suggestions of things to consider working towards under each of the four pillars: 

Employment

  • Build on strengths and interests highlighted in personal/vocational profile.
  • Apprenticeships, supported internships, and traineeships.
  • Further work on academic and vocational qualifications.
  • A-levels and planning for university.
  • CV writing.
  • Skills in applying for jobs or higher education.
  • Interviewing.
  • Understanding support from the Local Authority. For example, do they have a supported employment service?

Independent living

  • Managing bills. For example, a mobile phone.
  • Managing potential income, including Personal Independence Payments.
  • Residential and local learning options.
  • Mental capacity - supported decision making.
  • Understanding consent and best interest life skills.
  • Managing your time.
  • Transition to adult care.
  • Being safe in your home.
  • Understanding different types of living arrangements.
  • Planning for future living.

Community inclusion

  • Developing new friendships.
  • Personal Budgets.
  • Managing your time.
  • Being safe on the streets.
  • Understanding alcohol and drugs.
  • Volunteering.
  • Understanding the criminal justice system.
  • Knowing where to go for help and how to use the emergency services.

Health

  • Taking responsibility for dental and optical appointments.
  • Managing own health.
  • Transition to adult health services.
  • Knowing when you need to see the GP.
  • Staying physically active and healthy.
  • Understanding relationships, including sexual relationships, choice and consent, safety and good health.

 

Cognition and learning

Employment

  • Young person (YP) has built upon strengths and interests in their personal/vocational profile.
  • YP has achieved steps toward academic and vocational qualifications.
  • YP has achieved Level 3/A-level qualifications/equivalent to enable progression onto university or other education/training opportunities.
  • YP has developed their skills in CV writing and in applying for jobs or Higher Education.

Independence

  • Young Person (YP) is able to manage potential income. This includes personal independence payments (PIP) and incoming bills.
  • YP can show skills in time management and negotiating travel/transport.
  • YP understands different types of living arrangements and which of these are positive or possible.
  • YP has begun to plan for future living.

Community inclusion

  • Young person (YP) understands personal budgets and how to spend it to further PfA aspirations.
  • YP understands the potential risks relating to drugs and alcohol within the community. They will be able to make safe choices.
  • YP understands how the criminal justice system works, so they can function within the community.
  • YP has developed increasing social awareness. This includes understanding and reasoning skills to promote social and emotional wellbeing and reduce vulnerability within the community.

Health

  • Young person (YP) has an understanding of their health needs. They will be able to manage these where applicable.
  • YP can access their GP or other health professionals as appropriate.
  • YP has an understanding of the importance of regular checks including:
  • medical, dental and optical checks.
  • YP understands healthy choices. This includes healthy eating and the benefits of exercise. They will take steps to remain healthy and active. 

Communication and interaction

Employment

  • Young person (YP) has developed the appropriate communication and interaction skills to allow successful access to the following as required:
  • further / higher education, apprenticeships, internships, traineeships.
  • YP has developed the appropriate communication and interaction skills to allow the development of relationships with peers. These peers could be within the workplace/education environments and promote emotional wellbeing.
  • YP can show appropriate communication skills. Both written or verbal. This is to enable successful applications for jobs or higher education.
  • YP is able to respond appropriately to questions. They display the communication skills required to present their skills and attributes within an interview situation.

Independence

  • Young person (YP) has developed the communication and interaction skills to take part in residential and local learning options.
  • YP has developed the communication and interaction skills to facilitate independent living. For example, shopping and travel.
  • YP has developed the communication and interaction skills to enable them to discuss their views and opinions about future living arrangements.
  • YP can access information relating to travel and transport to facilitate independent travel appropriate to individual circumstances.

Community inclusion

  • Young person (YP) demonstrates appropriate communication and interaction skills to be able to access community, leisure and social activities.
  • YP is able to communicate their choices and preferences to ensure their wellbeing within the community.
  • YP shows appropriate communication and interaction skills to engage in voluntary work and/or community-based projects/initiatives.
  • YP is able to communicate with relevant agencies and /or emergency services.

Health

  • Young Person (YP) can access information relating to relevant health services to maintain good health.
  • YP is able to show responsibility for dental, medical and optical appointments. They communicate their needs and interact with appropriate staff to arrange these.
  • YP has developed the communication and interaction skills necessary (in the context of individual circumstances) to articulate health concerns/needs to relevant health professionals during appointments.

Social, emotional and mental health

Employment

  • Young person (YP) has acquired the necessary social skills to interact with employers and clients. They can function effectively in apprenticeships, internships and traineeships as required.
  • YP has developed appropriate communication and interaction skills to facilitate the development of relationships with peers within the workplace/education environments. This promotes emotional wellbeing.
  • YP demonstrates the ability to understand and manage their feelings and emotions. They are able to access appropriate strategies or help as required.

Independence

  • Young person (YP) understands their right to make choices and exercise decision making in relationships with others. Their emphasis should be on best interests and informed consent.
  • YP is aware of boundaries and social conventions in a range of relationships and social situations. This includes online.
  • YP recognises potential abusive and exploitative behaviour in others. They are able to make safe choices.
  • YP understands different types of living arrangements. They understand which are positive and possible in their own circumstances.

Community inclusion

  • Young person (YP) demonstrates the ability to understand and manage their feelings and emotions. They access appropriate strategies or help as required to facilitate/mediate interactions with others.
  • YP has developed appropriate social skills to establish new friendships in the community.
  • YP demonstrates awareness of social conventions and boundaries. They will be able to negotiate these to maintain personal safety while in the community.
  • YP has an awareness of boundaries and social conventions for a range of relationships and social situations. This includes online.
  • YP recognises potential abusive and exploitative behaviour in others. They are able to make safe choices.
  • YP understands the risks associated with drugs and alcohol. They adhere to legal restrictions concerning these substances.

Health

  • Young person (YP) engages with self-care routines to maintain personal hygiene. This also includes their environment.
  • YP makes safe choices in relation to sexual health. 
  • YP understands and manages their feelings and emotions. They access appropriate strategies or assistance as required to maintain emotional wellbeing.
  • YP employs strategies to maintain good mental health. This includes recognition of times when they are not coping and being able to seek help as required.

Sensory and physical

Employment

  • Young person (YP) can access and function within work-based environments to progress with future career choices. This includes apprenticeships, supported internships and traineeships
  • YP is able to to present their skills in written form (CV). This helps to secure future education and work-based options.
  • YP is able to present their skills verbally. They can answer competency/skills-based questions in preparation for job interviews.

Independence

  • Yoing person (YP) demonstrates the life skills necessary to facilitate independent living. For example, cooking, cleaning and shopping. This is demonstrated in the context of individual circumstances.
  • YP engages with self-care routines to maintain appropriate levels of personal hygiene per their individual circumstances.
  • YP has an awareness of risk within the home context and manages this to remain safe.
  • YP has planned for future living arrangements. They recognise what is positive and possible to promote independence and wellbeing.

Community inclusion

  • Young person (YP) can access community, leisure and social facilities. Enabling participation within the local community in accordance with the YP’s preference.
  • YP demonstrates the ability to access appropriate transport. This facilitates participation within the community, leisure and social activities.
  • YP shows awareness of risk in the context of community participation in order to remain safe. This includes travel, road safety and personal safety.

Health

  • Young person (YP) recognises the need for dental, medical and optical health. They take responsibility for making appointments as required.
  • YP takes steps to remain physically active and healthy in the context of their individual circumstances. 
  • YP makes healthy eating choices to promote physical wellbeing.
  • YP maintains any physiotherapy/occupational therapy regimes necessary to ensure physical health and optimum mobility. This is achieved within the context of their individual circumstances.
  • YP engages with self-care routines to maintain appropriate levels of personal hygiene per their individual circumstances.
