This page provides settings and professionals with essential forms for managing specialist equipment. The forms below are for:
- Requesting equipment for pupils following an assessment
- Reporting equipment faults
Following the assessment process, Inclusion Practitioners are now responsible for managing the equipment process. This includes ordering and fault reporting.
All assessments must be carried out by an appropriately qualified professional. For example, a Specialist Teacher or Speech and Language Therapist. This must be completed before any equipment request is submitted.
Please also ensure that you have read and fully understood your responsibilities as outlined in the loan agreement.

Use this form after an assessment of need has determined that specialist equipment is necessary to meet the pupil’s access or learning requirements.

Before submitting this form, please ensure:
- Basic troubleshooting has been completed
- Your setting’s IT team has reviewed the device
This aligns with our loan agreement, which authorises ICT personnel to carry out:
- maintenance
- repairs
- software updates
- security patching
- technical upkeep
