SEND Support Plan

Buckinghamshire Council have developed a SEND Support Plan for schools and educational settings. The SEND Support Plan is a co-produced document that outlines a child’s: 

  • unique needs
  • goals
  • strategies 

This is to ensure appropriate support and reasonable adjustments. 

The plan involves:

  1.  assessing the full range of needs
  2. setting SMART outcomes
  3. identifying tailored interventions and resources. 

The plan should be implemented, monitored, and reviewed regularly. Adapting as needs evolve and creating a new plan at key transition points or each academic year. It serves as a roadmap for those working with a child and a way to cost provision. For example, resources and interventions incorporate input from external specialists such as Specialist Teachers or Educational Psychologists to provide a holistic support framework.
