Employment

  • Young person (YP) consolidates or completes learning. They achieve outcomes to enable progression into:
    • further or higher education
    • employment
    • adult education or community learning
  • YP understands processes and support in relation to job centre provision.
  • YP understands and accesses benefits where applicable.

Independence

  • Young person (YP) continues to develop independent living skills through appropriate study programmes.
  • YP understands correspondence/bills and manages them appropriately.
  • YP has planned living arrangements in place.

Community inclusion

  • Young person (YP) shows awareness of the role of adult social care and can access the service as required. 
  • YP has developed increasing social awareness. This includes understanding and reasoning skills to promote social and emotional wellbeing and reducing vulnerability within the community.

Health

  • Young person (YP) manages health appointments/interventions.
