Employment

  • Young person (YP) can access and function within work-based environments to progress with future career choices. This includes apprenticeships, supported internships and traineeships
  • YP is able to to present their skills in written form (CV). This helps to secure future education and work-based options.
  • YP is able to present their skills verbally. They can answer competency/skills-based questions in preparation for job interviews.

Independence

  • Young person (YP) demonstrates the life skills necessary to facilitate independent living. For example, cooking, cleaning and shopping. This is demonstrated in the context of individual circumstances.
  • YP engages with self-care routines to maintain appropriate levels of personal hygiene per their individual circumstances.
  • YP has an awareness of risk within the home context and manages this to remain safe.
  • YP has planned for future living arrangements. They recognise what is positive and possible to promote independence and wellbeing.

Community inclusion

  • Young person (YP) can access community, leisure and social facilities. Enabling participation within the local community in accordance with the YP’s preference.
  • YP demonstrates the ability to access appropriate transport. This facilitates participation within the community, leisure and social activities.
  • YP shows awareness of risk in the context of community participation in order to remain safe. This includes travel, road safety and personal safety.

Health

  • Young person (YP) recognises the need for dental, medical and optical health. They take responsibility for making appointments as required.
  • YP takes steps to remain physically active and healthy in the context of their individual circumstances. 
  • YP makes healthy eating choices to promote physical wellbeing.
  • YP maintains any physiotherapy/occupational therapy regimes necessary to ensure physical health and optimum mobility. This is achieved within the context of their individual circumstances.
  • YP engages with self-care routines to maintain appropriate levels of personal hygiene per their individual circumstances.
